1) Please enter the date your patient will begin treatment in the “Date of Service” field
2) Next, please provide at least two of the following items regarding your patient:
• Member ID and date of birth (DOB) Or
• Member ID and first and last name
3) Then choose "Find This Member" to search for your patient.
