Entering an order request
Once registered, log in to the Carelon Medical Benefits Management provider portal to begin the order entry process. You will be guided through a series of questions regarding your patient, the requested procedure and your patient’s clinical condition.
- For questions related to a clinical request, contact Carelon at the number on your patient’s health ID card for further assistance
- Call the provider portal support team at 800-252-2021 for help using provider portal
- For other questions, please contact your local network relations consultant
