Carelon Comprehensive Home Assessment and Prevention Plan (CHAPP) Program

Carelon Medical Benefits Management is proud to announce its partnership with Anthem, Inc. for the CHAPP program. The CHAPP program is a comprehensive preventive care program designed to improve member experience and health outcomes through the provision of targeted health screenings and personalized preventive care coordination.

Carelon partners with Home Health Agencies to provide CHAPP program services in a member’s home during authorized home health visits.

Carelon utilizes CMS National and Local Coverage Determinations (NCD and LCD’s) when applicable, or Anthem medical policies and clinical Utilization Management guidelines to facilitate the appropriate evaluation of medical necessity by including assessment of the member throughout the continuum of care.
